Choosing the right abutment impacts the success and aesthetics of any dental implant. Abutments connect a dental prosthetic, such as a crown, and the implant post anchored in the jawbone. They are typically made from durable materials so the prosthetic stays securely in place, functioning as an artificial tooth root.

There are two main abutments: custom and stock. Understanding why these abutment types differ is crucial for giving your patients the best care for their dental needs. Let’s examine the differences between stock and custom abutments, focusing on their benefits and drawbacks depending on your patient’s situation.

What is a Stock Abutment?

Stock abutments are pre-manufactured and come in standardized sizes, making them convenient and cost-effective for many dental practices. While these prefabricated abutments offer significant advantages, they also have certain limitations that may affect the precision and aesthetics of the outcome.

Benefits of Stock Abutments

There are several benefits to offering your patients prefabricated abutments, including:

  • Cost-Effectivenes: They are less expensive than custom abutments, making them economical.
  • Time-Saving: Since they are prefabricated abutments, these components can significantly reduce the time required to complete a dental implant procedure for quicker restorations.
  • Versatility: Stock abutments are practical for various implant restorations, including tissue-level and bone-level implants.
  • User-Friendly: Their standardization allows for easier impression-taking.
  • Efficient Delivery: Prefabricated abutments are easily accessible, allowing your patients to receive timely treatment.
  • Functional Applications: These abutments are best for the posterior regions of the mouth, removable dentures, and screw-retained hybrid dentures.

Drawbacks of Stock Abutments

Despite their benefits, stock abutments come with several potential disadvantages that can affect the success and aesthetics of the dental implant:

  • Lack of Precision: Stock abutments are less customizable, which increases risk of complications such as peri-implantitis, an infection causing inflammation and bone loss around the implant.
  • Subgingival Margins: The margin of a stock abutment is often positioned subgingivally, making it challenging to check for the complete removal of all cement. This limitation can cause long-term complications, including inflammation and implant failure.
  • Limited Angulation Options: Stock abutments are typically available in straight configurations or with limited angulation options, so they may not align with the implant site's requirements for less-than-ideal profiles.
  • Crown Contouring Issues: The limited shape and configuration of prefabricated abutments can necessitate over- or under-contouring crowns for a better fit, affecting the restoration’s functionality and appearance.
  • Gingival Tissue Shaping: The gingival tissue naturally conforms to the shape of the stock abutment, potentially resulting in an unnatural appearance and insufficient support for adjacent gingival tissue.
  • Aesthetic Limitations: Prefabricated abutments are generally not recommended for use in the aesthetic zone since the margin placement is predetermined. This limitation over the placement makes it difficult to achieve the best possible results.

Stock abutments are typically made from titanium, the same material as most dental implants, ensuring compatibility and durability. While stock abutments offer cost-efficiency, ease of use, and versatility, they come with precision and aesthetic quality trade-offs. As such, they are more appropriate for non-visible areas and functional restorations rather than cases where achieving a highly aesthetic result is essential.

What is a Custom Abutment?

Custom Abutment Implants

Custom abutments offer a tailored solution compared to prefabricated abutments. Dental laboratories can create abutments that seamlessly integrate with your patient’s oral anatomy, offering exponential benefits for dental restorations.

Benefits of Custom Abutments

Custom abutments fit the unique contours of a patient’s mouth for a personalized and precise fit. Dental laboratories create abutments closely mimicking a natural tooth's emergence profile and shape by taking an impression of the implant position and surrounding gum tissue.

This customization provides several benefits:

  • Precise Fit: The abutments fit perfectly on the implant and support the gum tissue like a natural tooth, even as gums recede.
  • Durability: They can be milled to specifications from high-quality materials such as titanium, zirconia, or gold-colored noble metals.
  • Improved Crown Preparation: The customization options enable ideal crown preparation with corrections for proper angulation and a more lifelike emergence profile, giving you more control over the aesthetic outcome of the restoration.
  • Customizable Margin: The margin of a custom abutment can be set at the most aesthetically pleasing level, making it easier to maintain healthy gingiva and oral hygiene.

Like prefabricated abutments, custom abutments can be used with both bone-level and tissue-level implants, offering broad applicability.

Drawbacks of Custom Abutments

The primary drawback of custom abutments, especially compared to prefabricated abutments, is their higher cost. This cost increase reflects the additional steps required in the design and manufacturing process, which can make them less appealing to some patients and dentists. However, there are no clinical downsides to using custom abutments.

Custom abutments use durable materials such as titanium, zirconia, and even gold-hued noble metals. Although more expensive than stock abutments, they are ideal for restoring implants that rely heavily on aesthetic appeal, such as the six anterior teeth of either dental arch.
